Résumé This Regulation, based on the provisions of the Law on waste management, prescribe the form and content of the request for obtaining permits, as well as the form and content of the permit for performing the activity of waste landfill operator. The applicant shall submit the request for issuance of a permit for waste landfill operator on the form given in the Annex 1 which is part of this publication. This form has to be submitted in three copies to the Ministry of Environment and Physical Planning. Depending on the content of the submitted request, the necessary documents in accordance with article 4 of this Regulation shall be attached to the form. The application for a landfill operator license is attached the following documentation: decision for entry in the trade register; certificate of solvency of the applicant; certificate of the Public Revenue Office for paid taxes and other public duties; financial statements prepared in accordance with International Accounting standards for the last three years, with reports from a certified auditor; confirmation that no bankruptcy or liquidation procedure has been opened; confirmation that against the applicant with a final decision of a competent authority has not been pronounced any misdemeanor sanction, prohibition to perform activity in the field of waste management or prohibition to perform specific profession, duty or activity; certificate of financial guarantee or appropriate insurance, which would be sufficient for covering related costs of waste management and the cost of eliminating of possible harmful effects of such permitted activities whether occurred during or after the date of expiration of related license; confirmation that the legal entity has employed at least three people with higher education preparation in the field of natural and technical sciences; programme for work, monitoring and control as well as programme for closure and further care after landfill closure; statement of authenticity of the request, the given data and the documentation, signed by the applicant or the person authorized to represent (given on a form in Annex 2); receipt for paid administrative fees.
