Abstract This Regulation of the Minister of Environment, which is composed of nine paragraphs and five Annexes, establishes forms of lists containing information and data of environmental use and level of fees, and ways of presenting this information and data. The forms established by the Regulation are laid down in the Annexes. In particular, Annex 1 contains the form of the list of collective information of environmental use and level of fees. Annex 2 specifies the form of the list containing information on quantities and types of gases and dusts introduced into air. Annex 3 specifies the form of the list containing information on quantity and quality of collected groundwater, inland surface water and inland marine water.
