The Regulation is completed by eight schedules: Application for forest operator's licence; Forest operator's licence; Prescribed covenants terms and conditions of licence; Accumulated indicator points/period of suspension; Notice of Indicator Points Allocated; Notice of suspension of forest operator's licence; Notice of cancellation of forest operator's licence; and Notice of request for review.
