Abstract

This Regulation determines the form and prescribes all required contents of the document on the fish origin. This text further sets the manner of its issuance and completion. The document on the origin of the fish shall contain: - Inscription "Receipt of origin (first sale) of the fish", - serial number and date of issue of the receipt, - name of the legal entity, i.e. first and last name of a natural person - supplier, - headquarters of the legal entity, i.e. address of the natural person supplier, - fishing water where the fish production facility or the fishing point is located, - identification code of the supplier, - name and surname of the natural person, i.e. name of the legal person - buyer, - address of a natural person, i.e. registered office of the legal entity - buyer, - name and surname of the person carrying out the transport, - registration mark of the means of transport, - final destination of the fish, - date and place of loading, data on the type of fish, total weight, quality, unit price per kilogram total price for the shipment.
