Every application for a licence shall be on Form I in the Schedule hereto, and shall be addressed to the Licensing Authority. A holder of a licence shall notify the Licensing Authority if the vessel in respect of which the licence was issued ceases to be used in accordance with the terms and conditions of the licence.
