An authorization for the supply of a proscribed article shall be in Form 1. An authorized person shall, at the time of an authorization, give to the person to whom the authorization relates, the instructions of use as set out in Form 2. A pharmacist shall keep a record of the issue of proscribed articles supplied in accordance with an authorization in Form 3.
